This can happen multiple times in a day but it most commonly it happens in Conflict. I've had it happen once or twice after Shb launch in PVE content.I had this issue and it would cut out constantly and drive me insane in the new PVP mode, then I changed my "Listening position" setting back default (it was 13) and since I've had no issues with audio cutting out.
